Your experience matters. Your voice can drive change. Register

Over the course of 12 months, you’ll be asked to complete 4 brief surveys, each taking 5 minutes or less.

1. Let’s Get Started: set up your profile and tell us about yourself

2. Wellbeing Check-In: in 2 months, we will ask you about your abilities and daily life

3. Safety & Comfort Check-In: in 4 months, we will ask you about how you are managing daily safety and comfort

4. Annual Check-In: on your anniversary of registering, we will ask about your wellbeing, safety, and comfort so you can see where you have improved over time

These surveys give you the opportunity to share what matters most: why you chose to participate, how you use assistive devices, your physical function, fall history, and your overall health and well-being.

In return, you’ll gain valuable insights into your own progress over time—helping you better understand your mobility, safety, and quality of life. Your responses also contribute to a larger effort to elevate patient voices, support advocacy and policy efforts, and improve outcomes across the O&P field.

Thank You for Your Participation

If you choose to participate in the LOV survey, we thank you for choosing to share your experience with us. Your voice matters, and your participation helps improve understanding, care, and outcomes for the entire limb loss and limb difference community.
